Abstract

A six-month and two-old male infant with aortic origin of right pulmonary artery and patent ductus arteriosus is reported.
Aortic origin of the right pulmonary artery (AORPA) is a rare congenital malformation and it's natural history without surgery is poor with very high mortality during first year of life.
Both echocardiography. Cardiac catheterization & angiography and MRI were used in the diagnosis and preoperative assessment of the presence of pulmonary vascular obstructive disease.
Successful anatomic correction was perfomred by direct re-implantation of the right pulmonary artery to the main pulmonary trunk and ligation of ductus arteriosus.
Intraoperative lung biopsy showed early changes in both lung. Postoperatively measured PRV/LV was 0.4-0.45% and good hemodynamics.
